Position Purpose:

Disability Support Worker (DSW) role aims to provide person-centred support to participants, helping them achieve their NDIS goals and live as independently as possible. This includes delivering personalised care, promoting community participation, and ensuring the safety, well-being, and dignity of participants. The role also involves maintaining accurate documentation, following compliance protocols, collaborating with team members, and supporting the cleanliness and maintenance of participant living environments.

Key Responsibilities:

  • Provide person-centered support including personal hygiene, dressing, grooming, and mobility support.
  • Follow and adhere compliance, protocol and documentations
  • Communication and team collaboration
  • Assist with meal preparation, feeding, and dietary needs.
  • Administer medication in line with care plans and policies.
  • Facilitate community access and social activities to promote engagement.
  • Support with household tasks such as cleaning, laundry, and shopping.
  • Implement and contribute to individualised support plans.
  • Monitor and report on client progress, behaviours, and health changes.
  • Collaborate with families, healthcare professionals, and support team.
  • Maintain workplace health and safety standards.
  • Offer emotional support, companionship, and encouragement.

Key requirements

  • Minimum of 1 years of experience in disability or aged care support.
  • Certificate III or IV in Disability or Individual Support
  • Experience with high-complex needs, including mobility and behavioural support.
  • Understanding of person-centered practices and ability to work independently or in a team.
  • Strong verbal and written communication skills.
  • Ability to handle challenging situations with patience and empathy.
  • MUST have current First Aid and CPR certificate.
  • MUST have current police check
  • MUST have NDIS Worker Screening Check.
  • MUST have valid full driver’s license and access to a reliable vehicle.
